Below you will find a link to our waiver, along with additional data gathered during the membership application. We will have the forms at the space for new members, but feel free to download your own copy, and fill it out prior to coming. To ensure data security DO NOT email a completed form to us, we would much rather receive it in person.
Every new year you will be asked to fill out the waiver to ensure we have your current data, as well we use the opportunity to discover a little more about our members through a short survey. We have also included a media release form for you to fill out so that we can use the media we have created to promote the space. This is not a mandatory item, but it is very much appreciated.
